Enrollment Process
Enrollment time varies between 4 - 6 weeks depending upon how quickly the authorization request is submitted and approved.
Initial Steps
1. Complete Your Intake Application

Complete the client intake application provided on our website then submit it to our admissions email with the following supporting documents: a copy of your insurance card (front and back), driver’s license of parent/caregiver, a copy of the psychological evaluation, referral for ABA services from the primary care doctor (must have diagnosis listed in referral).
2. Eligibility of Benefits Check

Your insurance provider will be contacted to complete a check for coverage of applied behavior analysis services. This ABA benefits check will also include the client/caregiver financial responsibility estimate or total (e.g., out-of-pocket amount, copays, deductibles, etc. combined).

If the client/caregiver chooses to gain services through private pay (no use of third party payor), then an eligibility check is not needed and we will proceed to the next steps.
3. Schedule an Assessment

After all required documents have been received, the eligibility of benefits check is completed, and there is coverage for ABA services, the client/caregiver will be contacted to schedule an onboarding interview with the clinical director and observations of client.
Authorization
1. Request for Authorization

After the interview and assessment is completed, a clinician will complete an authorization request form and individualized treatment plan. The clinician will then meet with the caregiver to review the proposed plan and treatment hours requested. Upon consent for services, the clinician will submit to the insurance payor for coverage.
Schedule A Start Date

During the time of authorization request, and once approval is acquired, the clinician and caregiver/client will schedule the start of services.
