Privacy Policy

Personal Information: How It is Acquired, Utilized, Distributed, and Stored:

Joining the Great Strides collective, whether client, employee, or volunteer, requires collecting and using identifiable and non – identifiable personal information. Such information is collected by phone, paper, and/or online to carry out business purposes which include but are not limited to, registering clients and parents for database profile, checking for eligibility of benefits and applying for authorization of services to your insurance provider, and accepting payment for rendered services from the insurance payor and/or client. Because of this requirement and usage, it is imperative to be clear about the types of information collected and how it is used. 

The types of personal information collected and used include, but are not limited to:

  • General Personal Information (e.g., name, date of birth, home address, school address, email address, phone numbers, emergency contacts, etc.). This information is collected during the client intake process and employee hire and onboarding process. This information is used to create the client and employee profile.

  • Sensitive Information (e.g., clinical diagnoses, medical health history of client and immediate family, prescribed medications and dosages, any disabilities, race or ethnicity, etc.) is collected from both client and employee to ensure appropriate accommodations have been made for diversity and accessibility of both staff and clients.

  • Government – Issued Identification Numbers (e.g., social security numbers, driver’s license numbers, etc.) is collected from those seeking employment. This information is needed for completing state required background checks and payroll purposes.

  • Educational Information (e.g., enrollment status, grade school level, highest degree received, graduation year, school/university names, etc.) is collected through the client intake and employee onboarding process. This information is used to ensure education requirements for employees have been met as well as requirement for enrollment for clients has been met.

  • Professional Information (e.g., Registered Behavior Technician certification number and date, Behavior Analyst certification number and date, NPI number, CPR certification, Crisis prevention Trainings, etc.) to better justify equal opportunity employment and such qualifications for all prospective candidates. 

  • Financial information (e.g., credit/debit card information, bank account numbers, routing numbers, wire numbers, PIN numbers, etc.) is collected through client intake and employee hire and onboarding processes. Such information is used for payment to employees and accepting payment from clients for services rendered.

  • Other Information (e.g., pictures, videos, voice recordings, etc.) is collected through on-going treatment sessions, during scheduled events, trainings, and more. Such information would be used for advertisement and training purposes. However, both employees and clients have a right to opt out of sharing this information by signing “no” to the use of pictures and recordings for advertisement, training, or both during the intake application and onboarding process. 

 

How Great Strides Utilizes and Discloses Information:

Personal Information provided to Great Strides is used to apply and acquire authorization from services from commercial insurance payors for its clients via eligibility of benefits checks, authorization requests, and/or claim status checks. Information provided by employees is used to ensure qualifications for employment have been met and upheld. The owner of information is always notified and consent provided before personal information is shared. The owner of such information has the right to end the use and sharing of personal information; however, such may effect the owner’s ability to access services or employment from Great Strides Behavior Solutions, LLC. 

 

How Great Strides Stores and Protects Personal Information:

Great Strides implements reasonable security measures to protect against unauthorized access to or unauthorized modification, disclosure, and/or destruction of data. Except for employee directories and lists, and the sharing of information as set forth in this Privacy Policy, we restrict access to Personal Data to certain employees and outside parties unless their job obligations require information to operate, develop, or improve our services. These employees are bound by confidentiality obligations and may be subject to discipline, including termination and criminal prosecution, if they fail to meet these obligations. 

Personal Information is stored on a protected database and shall be held for seven (7) years after the end of services and employment from Great Strides Behavior Solutions, LLC. If the owner needs access to information after the work or service relationship has ended, a request sent to Great Strides administration can be used to gain access. However proof of identity and original ownership must be provided before distribution of information.
